Custom whitening trays and a starter kit of whitening gel for at-home teeth whitening and maintenance
Two complimentary tube of whitening gel **
Must remain a pt of the office to get full benefits
Teeth whitening trays that may need to be replaced (due to loss, breaks, etc.) at a reduced fee of $50 per replacement
